The Black LGBTQ+ Sexual Health & Healing Hub

A culturally competent, trauma-informed, sex-positive healthcare ecosystem designed specifically for Black queer and trans people who avoid traditional clinics due to stigma, racism, transphobia, and past harm.

The Hub operates as a centralized home for all sexual health, harm reduction, and survival-support services. It addresses health holistically by integrating medical care, spiritual wellness, community care, safety, and economic empowerment.

The following programs operate under the Hub:

1. Rapid HIV/STI Access & Body-Affirming Care

A low-barrier sexual health access point offering:

  • Rapid HIV testing

  • On-site STI testing & treatment navigation

  • Guaranteed linkage to care within 48 hours

  • On-demand PrEP and emergency PEP access

  • Hormone-friendly sexual health care (HRT + PrEP integration)

  • Body-positive, sex-positive educational materials created by Black LGBTQ+ educators

This service is intentionally designed for people who traditionally avoid hospitals and clinics, centering safety, confidentiality, and community trust.

2. The House Call Project: Mobile HIV/STI Care

A mobile clinic bringing sexual health to those who cannot—or will not—use mainstream systems.

Services include:

  • HIV & STI testing at shelters, hotels, encampments, and homes

  • Condom, lube, safer smoking/snorting kit distribution

  • HIV self-test kits

  • At-home PrEP starts and medication delivery

  • Transportation to medical appointments

This model meets community members exactly where they are, removing every barrier possible.

3. Black Trans & Queer Sex Worker Wellness Program

A stigma-free, decriminalization-informed program supporting Black LGBTQ+ sex workers through three integrated tracks:

A. Health & Harm Reduction

  • No-ID HIV/STI testing

  • “Bad date list” + community alert system

  • Safer sex kits tailored for different forms of sex work

  • Overdose prevention supplies & wound care

  • Partnerships with affirming clinics for specialized exams

B. Rights, Safety & Dignity

  • Know-your-rights education

  • Expungement support

  • Court navigation & accompaniment

  • Online safety/digital security

C. Economic Empowerment

  • Pathways to Tha Blacq Smoke job apprenticeships

  • Emergency cash fund for leaving unsafe situations

  • Financial planning & income stabilization for survival economies

This program protects sex workers’ safety while honoring their autonomy.

4. HIV Care Navigation & Retention Team

A specialized team for Black LGBTQ+ people living with HIV who have been underserved, misdiagnosed, or pushed out of care.

Supports include:

  • Appointment scheduling & reminders

  • Transportation support

  • Medication pickup/delivery

  • Peer navigators with lived experience

  • Healing justice practices (ancestral rituals, somatic support, grounding techniques)

This team restores trust and ensures long-term continuity of care.

5. PEP/PrEP 24-Hour Access Line

A crisis-access hotline staffed by Black queer and trans educators.

Functions:

  • Emergency PEP access within 72 hours

  • Same-day PrEP start navigation

  • Appointment coordination

  • Transportation support

  • Judgment-free sexual health counseling

6. Pleasure + Protection Education Initiative

A sex-positive curriculum designed for Black queer & trans youth and adults.

Modules include:

  • Pleasure-centered safer sex practices

  • HIV/STI prevention without respectability politics

  • Consent in queer and trans relationships

  • Safety on dating apps

  • Sex work harm reduction

  • Healthy relationships outside heteronormativity

This curriculum can be offered in schools, shelters, and community spaces.

7. Healing Justice Medical Respite Program

Short-term, supportive housing for individuals recovering from:

  • HIV-related complications

  • STI treatment

  • Gender-affirming surgeries

  • Violence, trauma, or crisis

Staffed by:
Peer navigators, harm reductionists, and healing justice practitioners who merge community, culture, and care.

8. Black LGBTQ+ Overdose Prevention & Drug Safety Initiative

An integrated harm reduction program recognizing that sexual health intersects with substance use.

Services include:

  • Naloxone distribution

  • Fentanyl & xylazine drug-checking strips

  • Safer smoking/snorting kits

  • Overdose prevention training

  • Sex work–specific drug safety support

  • Crisis de-escalation during high-risk encounters

9. Sexual Health Mutual Aid Fund

A flexible, rapid-response fund ensuring no one is denied sexual health care due to poverty or crisis.

Covers:

  • Clinic co-pays

  • STI medication

  • Transportation to appointments

  • Emergency housing during sexual health or safety crises

  • Emergency cash for sex workers avoiding unsafe clients

  • Safe hotel stays for recovery or crisis rest

This fund is an essential piece of community health and survival.
