NAVIGATE • Health, Housing, Community Stabilization & Case Management
A comprehensive system of healing-centered housing support, crisis response, and re-entry services for Black LGBTQ+ communities.
This pillar acknowledges the realities of displacement, incarceration, discrimination, and structural violence. It provides direct care, stability support, and pathways back into housing and community for those who have been excluded or criminalized.
1. Black LGBTQ Housing Navigation Hub
A specialized, anti-oppressive housing stabilization program designed for Black LGBTQ+ people who avoid shelters and traditional service systems due to discrimination or unsafe conditions.
The Hub uses a low-barrier, harm-reduction approach that honors autonomy and understands the realities of survival.
The Hub acts as a safe bridge from crisis to stable, affirming housing.
2. Liberation Health Access Line
A culturally rooted hotline created for Black LGBTQ+ people who need immediate, trusted support from people who share their lived experience.
This hotline centers privacy, respect, and non-judgment, filling the gap between institutional crisis lines and lived community needs.
This hotline ensures Black LGBTQ+ people always have a safe place to call — day or night.
3. Returning Home: Re-Entry Program
A re-entry initiative providing specialized support for Black LGBTQ+ people returning from jail, prison, or detention.
This program acknowledges the unique harm and systemic barriers faced by Black LGBTQ+ people who are incarcerated, and provides wraparound support rooted in dignity, autonomy, and community.
This program ensures that returning community members do not restart life alone — they return to a community that understands them, supports them, and invests in their long-term stability.
